Surv+ Membership Terms & Conditions

ACCEPTANCE OF TERMS

Surv+ memberships as of June 9, 2022

1. By agreeing to a Surv+ membership, I am implicitly agreeing to Surv’s General Terms and Conditions.  
2. Charges: I understand that as a Surv+ member, I will be charged the membership fee monthly to my card on file, on the same date as that which I initially enrolled in the membership. 
3. Usage:
   1. Time periods: I understand that the time period of “one month” refers to my unique billing date and period. Within that “month,” I will receive a credit up-to-$150 off my first job booked that month. 
      1. This credit can only be redeemed once, towards the first job within my billing month
      2. I understand that if I do not use this credit within my billing month, it will expire unused 
   2. Additional projects: I understand that any additional projects or jobs booked within my billing month (after my month credit has been redeemed) will incur the discounted Surv+ rate:  
      1. As a monthly member with our seasonal (3-month) package at $99/month, additional jobs within the month will receive a 10% discount
      2. As a monthly member with our 6-month package at $89/month, additional jobs within the month will receive a 15% discount
      3. As a monthly member with our 12-month package at $79/month, additional jobs within the month will receive a 20% discount
      4. As a monthly member with a custom package, additional jobs within the month will receive a discount as discussed in my initial Surv+ consultation. 
   3. Example: If my membership begins on June 8, I will be billed again July 8, then August 8. In my first “billing month,” between June 8 and July 8: my first job will be discounted up-to $150. My second job between June 8 and July 8 will be discounted by XX%, commensurate with my membership type. 
4. Cancellation: 
   1. I understand that per my unique membership, my commitment period may vary:
      1. As a monthly member with our seasonal (3-month) package, I am committing to keeping my membership for at least 3 billing periods before I can cancel
      2. As a monthly member with our 6-month package, I am committing to keeping my membership for at least 6 billing periods before I can cancel
      3. As a monthly member with our 12-month package, I am committing to keeping my membership for at least 12 billing periods before I can cancel
   2. I understand that no refunds will be given, and if I cancel my membership after being billed for the month, I can still use my credit within the month
   3. I understand that if I wish to cancel, I must give at least one week of notice to Surv before my next billing date occurs
   4. I understand I must communicate my wish to cancel via email to our team 
5. Special promotion: If utilizing a promotion to enroll, please keep in mind these are subject to additional stipulations including but not limited to: commitment periods, enrollment fees, etc. 
6. Custom membership: If my membership type differs from the monthly memberships described above, I understand additional stipulations including but not limited to: commitment periods, enrollment fees, etc. may still exist.

Surv+ memberships prior to June 9, 2022:

1. By agreeing to a Surv+ membership, I am implicitly agreeing to Surv’s General Terms and Conditions.  
2. Charges: I understand that as a Surv+ member, I will be charged:
   1. If monthly: the membership fee monthly to my card on file
   2. If annual:  the membership fee annually to my card on file 
3. Time periods: Regarding use of hours outside specific time periods, I understand:
   1. As a monthly member, if my membership includes 4+ hours of labor per month, I may roll one month’s hours over to use. I understand that with the exception of one single month, unused hours expire each month. 
   2. As a monthly member with ≤3 hours of labor per month, I may not roll any hour’s over for future use. I understand that unused hours expire each month. 
   3. As an annual member, I understand that within the year I must use all my hours, and that they will expire after the year ends. 
4. Additional hours: I understand that any overage of hours allotted in my membership during a given time period will incur the discounted Surv+ additional hour fee:
   1. As a monthly member with ≤3 hours of labor per month OR an annual member with ≤36 hours per year, additional hours exceeding my membership inclusion will be $35/hour. 
   2. As a monthly member with ≥4 hours of labor per month OR an annual member with ≥48 hours per year, additional hours exceeding my membership inclusion will be $30/hour. 
5. Cancellation: I understand that
   1. As an annual member, there is no cancellation nor refunds given. 
   2. As a monthly member, I may cancel my membership at any time. 
      1. If hours are unused in the month of cancellation, I will bank them for future use. The month will not be refunded.
6. If utilizing a promotion to enroll, please keep in mind these are subject to additional stipulations including but not limited to: commitment periods, enrollment fees, etc.
7. Your membership type (homeowner, enterprise, partnership) may involve additional stipulations including but not limited to: commitment periods, enrollment fees, etc.
